B1. ABR form

The ABR form is the general assessment and registration form. The ABR form must be completed online in ToetsingOnline.

After the form has been completed and submitted it must be printed. The signed hard copy version must be submitted to the review committee. The competent authority must receive the ABR form digitally (as pdf). The digital version does not have to be signed.
